Some player, like VLC, can load subtitle from independent subtitle file, while most popular video players, like Windows Media Player, QuickTime, iTunes, cannot.

Introduction of One Common Subtitle File Format - SRT Format
SRT file is a simple text file, its content looks like this:
00:01:00,000 --> 00:01:05,000
00:01:15,000 --> 00:01:20,000
The format is simple and straightforward. The file is consisted of several blocks separated by empty line, and each block contains 3 lines. In each block, the first line is a number begin from 1 and increase by 1 each time. The second line is show and hide time of the subtitle. The third line is the text of the subtitle.
